A form that reshapes per policy type, so enquiries arrive quotable

One generic “request a quote” form produced a queue an agent had to qualify by phone. The form was attached to intent instead.

Situation

  • Insurance brokerage lives and dies on lead quality.

  • Health, motor, home and travel policies each need completely different information before anyone can price them, and one generic form produced none of it.

Approach

  • The visitor picks a policy type first, and the form reshapes itself to ask only what that product needs.

  • Each path is short enough to finish on a phone, and the questions are the ones an underwriter would ask anyway — so what lands in the inbox is close to quotable.

  • The page is built for paid traffic: one clear promise per policy type, a single call to action above the fold, and a 24/7 support marker in the header.

Result

  • Fewer abandoned forms at the top.

  • Far less qualification work at the bottom.
